Counterexample
Let , the disjoint union of two copies of the real line. Identify with for every , and give the set of classes the quotient topology. The two classes of and of are the two origins. The space is locally homeomorphic to , second-countable and , but it is not Hausdorff.
Remarks
Removing leaves an open set homeomorphic to , and the same holds for ; these two open sets cover . Every neighbourhood of contains the image of for some , and so does every neighbourhood of . So any neighbourhood of meets any neighbourhood of , and the two origins cannot be separated. For the same reason the sequence of classes of converges to both and , so limits of sequences are not unique. This is why the usual definition of a topological manifold requires the Hausdorff condition: without it, this space would count as a -manifold.
